Card Holder Authorization

I, the above-named authorized credit card user, give Effortless Travel, and its representative, express authorization to charge my credit card for the purposes detailed above and any additional payments related to this trip on any date prior to travel. I understand that this form constitutes a legally binding contract and that by affixing my signature to this form, I will be held responsible for all agreed upon (as stated above) charges as well as any and all collection and legal fees. By signing this "Credit Card Authorization" form, I/my organization acknowledge receipt and understanding of its contents, including its "Explicit Authorization" clause. 

This payment is for travel as described and agreed upon. I understand that returns, refunds, and cancellations are different for each reservation and I have been advised of how they apply to me. I certify that I am an authorized user of 
this credit card and that I will not dispute the payment with my credit card company.
